This item is only obtained from the Roca's Death quest. The actual item oage has no information on how to obtain this item and to find out you have to search the forums. It would be great to add how to obtain it in the item's comment, if possible.

Stats will usually be different on any Instance gear. The stats for Instance gear vary from piece to piece. I have added a note on the item. Spoiler:
