All alchemist have a chance to fail whether or not they have lvl primary, junior, or superior. Even if an alchemist has max alchemy, the chance to fail will always be present, just not as much.
I know people who failed polar ultima a couple of times, these people being hardcore alchemist who know how to make things without looking at a recipe.
And for the record, I haven't failed any of the recipes I've seen at wlo labs except zircon, it had a massive chance to fail, was surprised I got the scarlet mace item.
